Unless the clock has 24 non-overlapping hours or the townsfolk see the clock being adjusted (which would break the illusion), they can't know if the clock is intended to be at AM or PM.
I suppose this isn't even getting into the subject of how gullible the townsfolk are to think the clock is accurate at all. You'd think these people have some slight memory of the time or look at the sky, but maybe the horologist is manipulating the clock and has caused mass insomnia, resulting in less critical thinking from the severely sleep-deprived townsfolk.

you can just add a rotating hand that does a full rotation every 24 hours
this way, if hour hand shows 12, but day hand shows 6, that means it's 12 pm
