I think the problem lies in the reporting system itself. The user is given an almost infinite set of options in the "reason" field.

Possible solution: reduce the options. Make the user select from a list of reasons why they are reporting the user. Include reasons in the list that aren't report worthy, and inform the user when they choose them. Sort of how Facebook's report system works.

Pictures aren't really needed.  We get chatlogs and worldIDs with reports.

Perhaps. It'd have to be fairly extensive though. I've wanted to report people in some games (like Guild Wars 2) for reasons that didn't fall into any of the categories but were still legit concerns.
