What happened to shift?

What i think that happens when there is is a new update:
All worlds are marked as "not updated".
All clients exit when the world is marked as not updated.
Everyone exits and the world is reseted and the not updated status is lost.

Now if there are some ghosts of players there...they will not exit and the world is still marked as not updated...

ghosts are just people stuck in a level who cant get out or in
