Not always, sometimes there is a cause, you just havent found it yet

yes but if bug happends with a chance of 1/100000 there is not much to fix because
1) you can't repeat it so it shouldn't bother you
2) you can't repeat it so we can't collect data of your actions to understand what's the problem
