messed up real bad on an interview to a highschool
signed up for their art major, but i wasn't very interested, so i billshutted the entire thing
and one of their requirements was a "portfolio" type exhibit, i just dug up some 3 year old pixel art made in microsoft paint, inserted them into a fancy powerpoint, and put it in a CD
i knew i feckud up when the other candidates had big ayy es es models, and one guy made a marble bust

When I was 6, my grandma's lawn chairs were stolen in the middle of the night. My parents decided to ask me if I had seen anything. I had slept through the whole thing, but with my 6 year old mindset I thought "o hecc I don't want to disappoint them". I knew a lot about cars back then, so I pretty much said "I saw a, uh, a red Toyota Tacoma", which was my next door neighbor's truck. He also had similar looking chairs, so he ended up getting arrested. I haven't told anyone to this day.
No, we never found the chairs either.

I haven't made the largest mistake in my life, yet.
I would probably say dropping out of high school, as per depression and anxiety, the usual.
Which, considerably, isn't too bad. I may finish it sometime this year, if I see fit, in regards to employment.
It's fairly annoying however, because despite taking several intelligence evaluations and surpassing my peers, I would still be required to waste my time further with pointless studies.

I was in a similar situation to Atilla (targeted As & A*s), I finished ~4 grades below my predicted grades in all subjects and a year after other people my age finished and I was in classes with people 2 years younger than me - as someone who doesn't talk (mute) it was a little distracting to be around a lot of new people who I knew were curious about me.
It sucked and didn't help my situation at all. I have a national gold award in maths - so I know I can do it but I only got a half qualification for maths core & mechanics at a D and C respectively.
My worst mistake was not fighting against depression, anorexia and self-harm at the start, I left it too late and damage has been done; I am slowly recovering and things are managabl.

So I'm rather excessive when it comes to forgetting big mistakes. Probably ties back into some freudian psychology. Some defensive mechanisms are rationalization, denial, projection. oh look. I thought I had to make one up. Nope, Freud also notes "Repression" which I'm pretty sure also applies to avoiding them embarrassing thoughts.
I'll let you know when I catch what my biggest mistake was. Food for thought: these topics ("biggest mistake? Best achievement?") say less about what you actually did and more about you as a person. What do YOU value in life, and are there times where you were bothered that you did or did not achieve it?
So, some people might value some physical feat or choice, others something social, or something education-related. Just throwing that out there.

I have this too. I can only remember very few things from when I was younger. 50% Of all my memory is from a year ago till now because that's when I finally got over myself. It makes it hard to get a clear image of who I was/am. So I guess my biggest mistake is refusing to remember stuff.
