1) So there is this ship. Well, every time a wooden plank starts to rot on the ship, they replace that plank with a brand new plank. If this continues for a few years, and every plank is replaced, is it still the same ship? Or is it a whole new ship? And to add to the confusion, what if all of the old planks where made into another ship? Which one would be the real ship? The one that had the planks replaced or the one made up of the old planks?

3) If I say, "It's raining outside, but I don't believe it is" does that make sense to you?

1. It is simply a matter of titles and state of being. The original ship would always be called the Ship of Theseus. and the new Ship would have a new name (if it had one).
Since everything is made up of matter that could be tracked back to the Big Bang, everything is essentially made from the original energy/matter. Technically humans gave things titles. The ship would keep its title and therefore be the same ship. Not only that

A river is not the land that makes up the river, the river is actually the water. So if you are being extremely exact, you cannot step into the same river twice. Sure you can chase the river flow but you still won't be in the exact H2O molecules that you were last time. Then, you can't step into the same river twice

Ah Bobithan you caught my inconstancy. After Dictionary.coming the meaning of river, I found that it has to be a large amount of water. Note that Heraclitus was referring to the smallest amount of water so this is not necessarily related to the original question.
