The reason that they dont change until you refresh is probably because you are cacheing the blocks

Only cache if they are not going to change

What the php file does is send you a header("Location: gray/empty.png"); so that the browser can fetch the cached blocks faster (hopefully).
Do you think that could cause any issues?

Um...
Im not sure about the specifics to do with PHP, but I would guess that maybe the browser is caching the redirect, in which case you might need to specify a specific status code (from a quick google search 302, 303, or 307 depending on what you want the browser to do)
Also make sure your dont have the cache control field set to public or private in the redirect, it should be fine if it is in the actual image response though

Youve got a typo on one of the blocks: The requested URL /eetrakk/big.htmll was not found on this server.

(Block at x=2, y=10, (0,0) being the bottom left corner)
Edit: and the ones to the right of / below it
