Deleted Portal still working
When removing a portal, for example say ID 1 and go through the portal with ID 2 target 1 you get teleported to the deleted portals coordinates,
even if there is a block there. Only fix is deleting the other portal (ID 2 of example). This is a one way thing though,
you won't get teleported when going to the location of where the portal used to be.

Happened to me before. It happened when using the duplication glitch. Try deleting the portal, saving your level, enter /loadlevel, and exiting and re-entering the level
