We do not know how someone gained access to their EE account. It could have been anything: typing it in on a public computer and leaving it open, telling someone they trust their password, inputting their password in a phising form, someone intercepted their plaintext HTTP credentials (MITM attack), could have used a very simple password (e.g. "password"), or, had a keylogger installed in which case anti malware/virus software should have taken care of it.

What's important now is to change passwords associated with EE and your email, and enabling two-factor authentication on the email account if possible.
